Day Services

  • Day Treatment - Supports individuals interested in a menu of services offered in a site based environment. Each consumer and his/her support team create a comprehensive and person-centered plan based on skills, interests and development needs. Enriched and highly trained staff offer individualized attention and maximum opportunity for consumers with significant disabilities and/or fragile health conditions. Activities are planned by the consumer and focus on community opportunities, volunteerism and inclusion as well as skill acquisition and participation in all aspects of daily living at the individuals highest level of independence. In addition, a full range of clinical supports is included in the menu of services as needed. These include nursing, physical therapy, occupational therapy, speech and language services, and behavior/psychology services. The Medical Director oversees each person’s plan of supports. The combination of clinical and habilitation supports are coordinated to fully enhance each individual’s wishes, desires and needs in a comprehensive and coordinated environment.

  • Senior Day Habilitation - Serves individuals that are interested in retirement and leisure opportunities. Community Inclusion is the primary focus of these groups. Activities are chosen by the participant and may include crafts, games, cooking, swimming, senior meals, volunteer opportunities etc. Senior Day Habilitation is available in a small community based site in Cobleskill as well as at the main campus in Schoharie. Individuals receiving day habilitation supports are able to access clinical services through New Dimensions in Health Care, Article 28 clinic, located conveniently in Cobleskill and Schoharie.

  • Young Adult Day Habilitation - Serves individuals, transitioning out of school, who are interested in continuing to work on academic skills, social skills, and enjoying opportunities for community inclusion as a prerequisite for future employment and in preparation of adult life. Participants spend their days engaged in many activities both in the community and at the day treatment center.

  • Transitions Day Habilitation - Serves individuals of various ages with a variety of interests and abilities. Opportunities for leisure and volunteer activities in the community as well as communication, social, sensory and life skills are provided in small, individualized groups at the day treatment center.
